Identify a safety issue you have concerns about in clinical. Describe the issue and data
you can obtain; then analyze the data and plan to improve care related to the issue. Share
your information in class. This may be done in teams with different team members

Interview five staff nurses, and ask them how they know when quality care has been
provided. Summarize the data and compare with data your classmates collected. What are
the trends? Differences? Develop a table to describe the data. (Online Application

Go to the Institute for Healthcare Improvement website, and learn more about FMEA,
FTR, RRT, handoffs, and work-arounds. How might these methods apply to the
healthcare organizations where you are for clinical? You might do this as a team and

Discuss the use of root cause analysis in a team discussion. What is your opinion of this
process? How do you think it could be used by nurses and by the interprofessional team?
