Overview: The American Academy of Nursing (AAN) praises the Centers for Medicare
& Medicaid Services (CMS) for its support of evidence-based care coordination and
transitional care, which has been applied to Medicare and Medicaid services. The AAN
recommends that the CMS consider the framework it will use to implement
care coordination and the evidence to support that framework.
