Instructions:
Using the form provided, assess, and describe the contributions to the project of each member of your group.
Consider the member’s demonstrated ability to meet the five criteria listed in the rubric on the next page. While
assessing a member contribution assign a grade between 0 and 4 for each criterion and calculate the total.
Give a brief explanation as to why you graded the member the way you did.

Requirements:
1. Every student must evaluate each group member’s performance toward the completion of any of the
group assignments.
2. Peer Evaluation is confidential and must be submitted individually and not to through the group’s file
exchange
