Introduction
Today, a hot forging object fell on a worker, causing serious injuries and bodily
suffering. The incident occurred on an original equipment manufacturer with 300 employees that
lacked a safety specialist on staff. The day-shift superintendents are in charge of general safety
practices. Furthermore, no previous occupational hazard analysis has ever been completed. The
manufacturer made an effort to stop workers from acting in a hazardous manner by using posters
and supervisor instructions to remind staff to work safely. Nevertheless, today a serious incident
that occurred caused the worker to lose vision in his right eye, most of his arm, and right ear.
There will be an incident investigation conducted by authorized person. The purpose of the
incident investigation is to determine how and why these failures occur and prevent more
