Introduction
On June 7th, an employee was injured on the job while operating a table saw. The intent of
this report is to determine 1) how the incident occurred, 2) who was at fault, 3) how should this
incidence be addressed, and 4) what can and should be done to prevent or limit accidents of this
nature from occurring again in the future.0
Explanation of Issue/Problem
On June 7th, John Schmidt was severely injured at work in the production shop while
operating the table saw. Harry Hiller, the foreman, provided documentation showing the
