COVID-19 International Travel Request Checklist
Currently, all university sponsored/affiliated out-ofstate and international travel is restricted.
Exceptions may be made for faculty and staff who plan on traveling to participate in essential, low-risk
activities only.
Essential Travel Definition: Time sensitive, and critical to the mission and function of the department.
Travel may be proposed by individual travelers or by their supervisor. If sufficiently articulated in the
proposal, ongoing travel need only be proposed and approved once.
1. Proposer must write a proposal that includes the following information:
a. Traveler(s) COVID vaccination status
b. Description of type of activity traveler(s) will be engaging in
c. Explanation of rationale/critical need for a travel exception
d. Detailed itinerary including mode of travel/transportation, dates, and location
e. Describe familiarity with the proposed destination
f. Provide details of COVID-19-related risks at destination and describe personal risk
mitigation plan for keeping traveler(s) safe and socially distanced
i. Note: Traveler should review country-specific travel guidance and regulations as
they may vary greatly from MA state guidance. Example: Travelers arriving at a
specific country may be required by that country to produce a negative test,
even if the MA guidance does not require it.
