Line 1 is supervised by George Adams who is well respected by his men. One Wednesday
morning it was observed that nearly 10% of the quarter panels coming off line 1 were rejected
by Quality Control because of burrs and rough spots in the panels. It was reported that the
problem occurred three minutes after the end of the 10 o’clock relief break. When asked line
advisor about the rejects he said that he had no idea what was causing the problem. Engineering
department claimed there was nothing wrong with presses and the blanker. When checked a
few hours later at 4 o’clock, line 1 was producing 9 or 10 rejects an hour.

Line 2 is supervised by James Farrell who gets angered easily and is ambitious. On Wednesday
morning line 2 also started producing 10% rejects along with line 1. According to Quality
Control line 2 started producing rejects at 9:30 AM. Line #2 supervisor said that he checked
the sheets before going into the press and found nothing suspicious. When asked what
according to him might be causing the burrs he replied that he had his suspicions on the
drawpress operators but did not have any proof to support his allegations. When Farrell asked
his press operators to properly place the sheets in the press to avoid the rough spots, the
operators blankly stared at him. When checked at 4 o’clock line 2 was producing about a dozen
ruined panels.

Line 2 is supervised by Henry Dawson who is a warm-hearted person and is genuinely liked
by his men. On Wednesday morning when line 1 and line 2 were producing rejects line 3 was
producing perfectly good panels. After 11 A.M. when line 4 also started producing rejects it
was expected that line 3 would also soon start producing rejects. According to the line
supervisors, line 3 was not producing rejects because it had only one stack to start off while
other three lines had multiple stacks to start off. But according to management line 3 was not
