Background:
Jack is a 19-year-old, birthday November 10, 1994, Caucasian male who
was born and raised in Shawnee, Kansas for fourteen years and then moved to
South Africa for four years. After four years in South Africa he moved back to the
same house in Shawnee, Kansas. Jack is currently engaged to his future wife
whom he has been dating for 3½ months and they have moved in together with
her 2-year-old son and are now residing in an apartment in Olathe, Kansas. The
neighborhood is primarily low income African American population (subsidized
housing). Jack is currently employed, working two jobs at 70 hours a week. He
showed up to his therapy session wearing a nice polo, black slacks, and
professional shoes. His hair was combed and he was clean-shaven.
Jack has a history of emotional and physical abuse by his parents from a
young age and he has emotional abuse from his past relationships. This abuse
has led him to alcoholism and drug abuse. His preferred drug is marijuana, but
he has dabbled in acid, mushrooms, and oxycodone. Jack has seen a therapist
in the past (08/23/2005-03/17/2007) because he was experiencing suicidal
thoughts, Jack admitted he does experience them still at times. Jack is feeling
that he is going to relapse because he has a lot of stress in his life; he also feels
