AFFIDAVIT OF CORRECTION
That I, JONATHAN PINO DEVERA, Filipino, of legal age, single and a resident of
Purok Progressive, Maribulan, Alabel, Sarangani Province, Philippines after having been
duly sworn to in accordance with law, do hereby depose and state:
1. That I am one and the same person who made this affidavit to be prepared;
2. That I am a member of the Philippine National Police as Police Officer 1
with badge number 283451 under Regional Public Safety Battalion, General Santos City;
3. That I discovered, however, in the General Orders Number 2017-09 dated
January 16, 2017 for Declaration of Completion of the Public Safety duly issued by
Regional Training Center 12, Camp Gregorio D. Daproza, Lanton, Apopong, General Santos
City, my badge number was erroneously entered and reflected as 283444 instead of
283451 as can be gleaned in my pay slip dated February 2017, hereto attached for your
perusal;
4. That in order to avoid confusion, I now categorically state under oath that
the badge number 283444 as reflected in the aforesaid document and the badge number
reflected in my pay slip as 283451, pertains to one and the same person;
5. That I am executing this affidavit to attest to the truth of the foregoing
statements and for all legal intents and purposes it may serve.
