Diagnosis:
Bipolar I Disorder Severe 296.43 (F31.13) with Catatonia 293.89 (F06.1)
To meet the criteria for bipolar I disorder the DSM-5 requires the client to meet criteria for a
manic episode. This client meets criteria for a manic episode as follows:
The first diagnostic criterion for a manic episode is that the client has at least a one-week period
of abnormal and persistent elevated, expansive or irritable mood that is present for most of the
day, almost every day. This client has elevated his sexual demands on his girlfriend and he also
spontaneously starts conversations with strangers about his newfound religion while he is
hospitalized. Also the client must experience an abnormal and persistent increase in activity or
energy for a period of at least one week, and be present for most of the day, almost every day.
This is evident in the client’s increase in frequency of going to the gym to burn extra energy.
Secondly, the client must exhibit at least three of the listed symptoms in the DSM-5 during the
period of mood disturbance and increased energy and activity. The symptoms must be present to
a significant degree and unusual to the individual. This client experiences five out of seven of the
symptoms as described below:
1. Inflated self-esteem or grandiosity is apparent in the client’s claims that he is directly talking
to God.
2. The client has stopped sleeping and yet reports having excess energy to burn, which shows his
decrease in need for sleep.
